In 2010, CJ’s Thumbs Up Foundation will distribute twenty (20) $500 gifts ($10,000 total) to families in need. These financial assistance grants can be used for any necessary expenses on the part of the family.
o The applicant must be a child (up to 18 years of age) currently served by VCU Childrens Medical Center (MCV) who has a documented chronic and life threatening illness.
o We will have two distribution periods in 2010
o 1st deadline for application May 1st; awards given by June 1st
o 2nd deadline for application November 1st; awards given by December 1st

o If more applications are received than awards are available, applications will be kept on file and place will be “held” until next application period ends. Applicants will need to submit a renewal application that confirms that financial need continues. This will need to be signed by a physician.
o Families may receive no more than 1 gift per year and no more than 2 gifts per lifetime.
